Care Coordination for Children provides case management for children and families with special development needs.

• Identifies children at risk for learning and growth delays, disability, and chronic illness in order to help provide access to services that will allow them the maximum opportunity to reach their developmental potential.
• This service is available to Iredell County residents birth to age 5 who are at risk or have diagnosed developmental delay/disability, chronic illness or socio-emotional disorder.
