Care Management for High-Risk Pregnancies (CMHRP)

The Care Management for High Risk Pregnancies (CMHRP) program provides case management services to Medicaid-eligible women who may be at risk for adverse birth outcomes. 

CMHRP Care Managers work closely with the woman’s OB provider to ensure their needs are met to promote a healthy pregnancy and positive birth outcomes.

Examples of services provided are:

  • Referrals to WIC and other food/nutrition programs
  • Assistance with obtaining services such as transportation, medical care, and stable housing
  • Support with following your prenatal care provider’s plan for a healthy pregnancy
  • Providing information on childbirth or breastfeeding education classes
